if you are applying for a Visa at another consulate, please check the guidelines where your jurisdiction is
I Recommend sending the ROM and applications for PH VISA also to the consulate in San Francisco
(Faster process, Please check My cover letter How I do that)

If you are from Europe or another country
check in your consulate what is the required documents.
